UHN Educational Workshop: Long-term Care Planning

The University Health Network is holding a series of Dementia Care Partner Educational Workshops. The third session is about long-term care planning with Dara Zarnett, Manager for Placement, Toronto Central Local Health Integration Network, Home, and Community Care.
